Integrating Pharmacotherapy into Ambulatory Psychiatry: A Pilot of Team-Based Care to Develop the Pharmacist, Psychiatrist and Patient Triad

We will develop an entirely new system of care integrating the services of a clinical pharmacist into the Adult Outpatient Psychiatry service. We will build out patient-centered, team-based care to improve psychiatric medication management. Our team vision is a partnership between the patient, primary psychiatric provider, and clinical pharmacist to:
1. Identify and support patient goals of care with wrap-around support of the triad;
2. Grow patient knowledge about psychiatric medications, lifestyle modifications, and psychological tools to improve mental health;
3. Empower patients to set goals of care and become experts in their own treatment;
4. Measure the value of our changed systems by bringing metrics and measurements into our workflow; and
5. Develop a population health approach in Psychiatry by creating patient registries that identify and track areas of quality improvement.
